Gram – CSF: neutrophils, no bacteria

HISTORY

A simulated CSF smear collected from a 16 year old female patient with headache and stiff neck was sent to category A, B, C, and C1 labor-atories for Gram staining.

Participants were expected to report the presence of neutrophils and no bacteria.

MAIN EDUCATIONAL POINTS from GS144-2

  1. Heavy neutrophils, without organisms seen on the Gram stain, may suggest a number of different diagnoses, and should be re-ported to the clinician as soon as possible. The lack of organisms does not rule out bacterial meningitis.
