Gram Smear:  CSF: gram positive bacilli and neutrophils  – Shipped May 2013

HISTORY

A simulated CSF smear collected from a 3 day old baby with sepsis was sent to category A, B, C, and C1 laboratories for Gram staining.

Participants were expected to report the presence of gram positive bacilli and neutrophils.

MAIN EDUCATIONAL POINTS from G131

  1. As previously highlighted in Challenge GS114-1, the reporting of epithelial cells when none are present is significant because it suggests a problem of competency in reading and interpreting cells.
  2. The Gram stain of CSF from patients with L. monocytogenes meningitis will be positive in roughly 25% of patients – less than half the number of positive Gram stains in patients with meningitis due to other bacterial agents.
