Wound: Nocardia species  – Shipped February 11, 2019

HISTORY

A simulated wound swab collected from a 58 year old leukemia patient with subcutaneous abscess was sent to category A laboratories.

Participants were expected to report Nocardia species.

 

MAIN EDUCATIONAL POINTS from M184-3

  1. Nocardia species may be missed with routine laboratory methods since it can require extended incubation and/or selective media for detection; therefore, beading or branching gram positive bacilli in the direct smear can be key to the diagnosis.
  2.  Nocardia is an opportunistic pathogen of great concern in immunocompromised patients due to the increased incidence of dissemination.
