RESISTANCE TO MACROLIDES AND LINCOSAMIDES

Macrolides have been in use since the early 1950s and are a major alternative to the use of penicillins for the treatment of infections due to gram-positive microorganisms. However, resistance to erythromycin emerged as early as 1955. The rapid increase in incidence of macrolide resistance makes its surveillance necessary.

Macrolides are composed of 2 or more amino or neutral sugars attached to a lactone ring of variable size. Commercially available macrolides have a 14-membered (clarithromycin, erythromycin, and roxithromycin) or 15-membered (azithromycin) lactone ring. Lincosamides (clindamycin and lincomycin) are devoid of a lactone ring.
